Note: We will do our best to honor all group requests; however, it may not be possible to place your child in the same group as one of his or her friends.
By submitting and signing this form, I/we, the legal guardian(s) of the aforementioned child(ren), grant permission for my/our child(ren) to attend and fully participate in Delaware Bible Church’s 2026 summer Vacation Bible School program.
I/we, on behalf of my/our child(ren), hereby release, forever discharge, and agree to hold harmless Delaware Bible Church, its directors, employees, and agents from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses of any kind, which may be incurred by the undersigned or the child(ren) while participating in Vacation Bible School.
Furthermore, I/we assume all risks of personal injury, sickness, death, damage, and expenses resulting from participation in Vacation Bible School. I/we agree to indemnify and hold harmless Delaware Bible Church, its directors, employees, and agents from any liability arising from negligent, willful, or intentional acts committed by my/our child(ren), including any expenses incurred as a result.
I/we, the parent(s) or legal guardian(s) of the child(ren), grant permission for him/her/them to participate fully in all Vacation Bible School activities. I/we also authorize Delaware Bible Church to administer basic first aid and to contact emergency medical services (911) in the event of a medical emergency. I/we accept responsibility for any and all resulting medical expenses.
